The SR code is not properly filling out the segment list. There's three key issues:

• The first segment needs to remain in the segment list; at the moment it's popping it from the list and using it only as the (initial) destination addr.
• The segment list template needs a space for the ultimate destination.
• The ultimate destination needs to be inserted into that space when adding the SRH.
